Printable bird tag

Fill it out, sign it, put it in the box.

Federal law requires a signed tag on any duck or goose left with a taxidermist, showing the hunter's address, the number and species of birds, and the date they were taken. Turkeys and upland birds don't need it federally, but send one anyway so I have a record of your bird.
